Application for Leadership Blair County Youth is a two-part process. *Please note the distinctions between where each form is to be returned.
Step 1: Download Application Form
- To begin, a student fills out an application form. Instructions for filling out the form are on the document. Remember, do not use pencil, and do have the form signed by a parent or guardian.
- Please return completed application to your guidance counselor by April 21, 2008.
Step 2: Download Referral Application
- 2 application referrals must be completed and sent directly to The Chamber. Non-family members, i.e. teachers, guidance counselors, employers, members of the applicants church etc, should complete these forms. In order to assure the integrity of the form they must be returned directly to The Chamber by the referrer. Instructions for this are provided on the form.
- Application referrals should be returned to The Chamber no later than April 21, 2008.
